Healthy Kids Colorado Survey

The Healthy Kids Colorado Survey will ask students about health topics including: exercise, diet, alcohol, tobacco, drug use, mental health, suicide, bullying, sexual behaviors, and how they have been affected by the COVID19 pandemic. The survey also asks students about school-life, school safety, trusted adult relationships, and other things known to be connected with healthy choices.

In order to get the most accurate information about youth health, we would like all students in grades 6th through 12th to take part in the survey, but students do not have to take this survey. The survey is optional and anonymous. It takes no longer than 45 minutes to complete. The survey process will protect students’ privacy. No information is collected on the survey that can identify a student. Only the student will know their individual responses. Students do not get school credit for taking the survey, and there is no penalty for not taking the survey. Students who take the survey may choose not to answer any question.

Parent/guardian letters, including opt out forms were given to students to take home the week of Sept 20th. Here is the Parent/Guardian Letter. If a student is opting out of the survey, we ask for their parent/guardian to return the opt out form or contact the school by October 7th.

The survey results will help plan programs and services for state health and human services agencies, Ouray county schools and community groups, including: Voyager, Communities that Care, Sources of Strength and Safer More Affirming Communities.
