Immunization Information

The State of Colorado and Ouray School require students to meet the standard for immunization for communicable diseases. The following are required by the Colorado Department of Public Health and Environment.
  • Hepatitis B.

  • Diphtheria, tetanus, pertussis (DTap).*

  • Inactivated poliovirus (IPV).*

  • Measles, mumps, rubella (MMR).*

  • Varicella (chicken pox).*

  • Tetanus, diphtheria, pertussis (Tdap).**

*Students 4 through 6 years of age are required to have their final doses of DTaP, IPV,MMR and Varicella prior to kindergarten entry.

**Students are required to have Tdap prior to entry into 6th grade. One dose of Tdap is required for 6th through 12th grades.

 

The school district is obligated to support the public health efforts while respecting a parent's right to exempt their child from vaccine and immunization requirements.
 
Per state law, Ouray School provides families with the standardized certificate of immunization and publishes school vaccination rates. Additionally, the school provides resources for families regarding changes to school immunization requirements in 2021 with respect to exemption criteria.

Immunization Exemptions

No student is permitted to attend or continue to attend any school in this district without meeting the legal requirements of immunization against disease unless the student has presented one of the following, as provided by law:
  • a written authorization signed by a parent/guardian requesting local public health officials administer the immunizations;
  • a certificate of medical exemption;
  • a certificate of completion of the online education module administered by the Colorado Department of Public Health and  Environment*; or
  • a certificate of nonmedical exemption*.
*These exemptions require annual submissions to the school.
 
Students who do not have the required immunizations or submit one of the state exemptions above will not be allowed to attend school. Please see the accompanying Ouray School Board Policy in the right column of this page.
 
All information distributed to parents/guardians by the district will inform them of their rights to seek an exemption from immunization requirements.

Immunization Rates

Per Colorado law, Ouray School is required to publish school immunization rates with a goal of 95%. While this is the target, Ouray School is not responsible or accountable for meeting this rate. Further, the school is required to publish exemption rates.
